Can someone point me in the right direction as to where it states that a skin form must be dated in the last seven days??

I understand that USAWKS requires a providers script pad or letterhead. The form itself has an expiration date and date treatment started, but I cannot find it in the by laws or modifications that it must be dated in the last seven days.

Tournament flyers, Sub-district flyers and District flyers are the only places I see that.

Chances are I missed it! But I'm tired of looking!

BTW. I don't have an issue with it, just a topic that has come up.

I had asked this before and when USAWKS adopted the new skin form the requirement of a 7 day renewal went away.

I told my parents that if you think you have a questionable spot, to go ahead and have it renewed just to be safe.

Good question because I think there is still confusion around the 7 day requirement.

I think the old form stated the 7 days where as the new one doesn't.

Yes, when in doubt get a form for sure!

But I'm curious if this is a real requirement, or just old fliers being copied from year to year??
